5 Things To Consider Before Joining Courses At Any Dental Academy

5 Things To Consider Before Joining Courses At Any Dental Academy

Now a days as the level of Dental Education in some Indian Dental Schools is all time low.  There are some responsible experienced dental professionals who have come forward to guide young dental graduates. But unfortunately not many are able to stick to the original motive of imparting dental education and it is becoming more of a kind of business.

To buy or sell second hand dental clinic equipment visit the dental portal DentistFriend.com.

Irony is that these days one has to take coaching for cracking premedical entrance exams to become a doctor and later they have to again take coaching even after graduating as Dental Surgeons.

If you go to any dental event or dental conferences and you will find some people standing at the entrance of venue distributing leaflets of various Dental Academies. The most funny/sad part is that some of these academies are being run by freshly pass outs and they are teaching subjects which need vast experience of over the years to master. PLEASE BEWARE OF THEM.

For example, How come you expect to learn Oral Implantology from someone who himself has not placed even 100 implants? Does he know the failures? As they say, Master has failed more times than the beginner has even tried.

My point of writing this blog is to share my personal experience that may help you in deciding from which right place to learn. Definitely we all need to learn and upgrade ourselves from time to time. So, here are the top 5 points to consider before joining any academy:

1.  Get reviews from your friends, college teachers, internet and check yourself which one is most liked and reasons behind it and then only make a decision. Never make a decision to join any academy considering your convenience rather give priority to best learning experience. Learning will never come easy, be always ready to leave your comfort zone, no matter even if you have to travel PAN India.

2.  Don't join academies mentored by any Novice. A novice can teach in right fashion and may know the Ideal work but he won’t be able to teach you the failures for sure as he himself is not aware as have not encountered many in his short tryst with the subject.

For example: Learn Oral implantology from a mentor who has done more than 1000-1500 implants over 10-15 years. i.e. now he/she is a speaker of International repute. He/She has seen the failures and will be able to teach you how to overcome them.

3.  It hardly matters whether your mentor is a BDS or MDS. This should not be your criterion of selection. Count the number of years of their practice and not degrees. Degrees do have a value but they don't matter much in this aspect. Many big names in this field are old time plain graduates who have done unofficial masters in clinical procedures at their own clinics.

For instance, one of my good friends is a pioneer in teaching micro-endodontics in a North Indian city near National Capital. He himself is a BDS graduate but has many endodontists in his fan list.

4.  Don't go for fancy courses, particularly Orthodontics courses. No doubt a BDS can practice orthodontics but that is possible after proper orthodontic training. Ortho is a very vast science in which prognosis is majorly based on treatment planning & how can you even expect to learn treatment planning in 5-10 days? Which otherwise needs 3 years.

I recently met someone who was repenting of wasting his energy and resources for doing a 7 day short ortho course and finally he has to call an Orthodontist for consultation.

5.  Lastly and most importantly. Choose the courses in which you have more number of patients to do than just the theory part. You always have Google option for theory and demo Videos.

I hope these points will help you in your selection of right course and academy.

Please Note: Further inputs from seniors in profession and friends are most welcome.

Clinical Dentistry: Should Dental Professionals Practice Botox & Dermal Fillers?

Clinical Dentistry: Should Dental Professionals Practice Botox & Dermal Fillers?

A lot is being talked and argued about the administration of Botox by dental professionals in India. Many dentists believe that Botox and Dermal fillers belong to dermatology and dentists should not perform these procedures; which is actually a misconception. This misbelief is because not many dentists in India are aware of the significant benefits that Botox has as an adjunct therapy in many dental procedures.

Use of Botox in dentistry is relatively a new discovery, so probably its acceptance will take some time among Indian dental professionals. If you are a dentist practicing in India and have the same misconception then this blog is for you. I am hopeful that after reading this article your false impression will fade away to some extent.

The first thing that I want all the dentists to realize is that we are part of the medical sphere and we dentists are “real doctors.” Every time when we inject LA into a patient, we are delivering a medicinal agent into the human body that has real systemic complications. The only difference is; we have received special training and mastered to deliver local anesthetic into the human body and are comfortable in dealing with the complications. By the way, I would like share a fact that the adverse reactions and complications associated with the LA that we use every day in our dental practice are far more serious than those with Botox and Dermal fillers.

After training hundreds of dentists in India; I can say that, with proper training on Botox & Dermal fillers even you can become equally comfortable in administration of Botox like you are with local anesthetic. In fact I believe that we dentists can do it better in relation to other medical professionals when it comes to head-neck-face region as we are specialized in it.

Here are some dental treatments where you can use Botox & Dermal fillers:

1. Temporo Mandibular Joint Disorder

2. Bruxism

3. Facial Pain

4. Gummy Smile

5. Full Mouth Rehabilitation

6. Smile Designing

All the problems that I have mentioned above fall in the realm of dentistry and dentist. I believe, no other medical professional can do justice to such dental originated problems except a dentist. So let's not take these issues as a profession and stop letting politics get in the way of science & common sense. If you still feel that Botox and Dermal fillers are not for dentists then you seriously need to re-think!!!

I will share some of the cases that I have treated with Botox and Dermal Fillers in my upcoming blog posts.

What is botulinum toxin?

Botulinum toxin is produced by gram-positive anaerobic bacteria Clostridium Botulinum. Botox is a trade name used by Allergen for Botulinum toxin and basically it is a purified form of botulinum toxin which has both aesthetic and therapeutic uses. The trade name Botox has become so popular that people recognize trade name better than its actual name; just like we use the trade name Xerox for photocopy. Many of us get afraid to hear the word toxin but botulinum has the real role to play in dental treatments. It is the toxin that helps to age gracefully.

Are there any variants of Botulinum Toxin?

Yes, there are seven type of botulinum toxins which are available to us. It is from type A, B, C, D, E, F & G. The one which is used most commonly is type A; however if any individual gets resistant to type A then we can use type B botulinum toxin. The type B botulinum Toxin is known as Myoblock.

Botulinum Toxin is available to us from different pharmaceutical companies.  There are several other trade names of botulinum Toxin like Desport, Neuronox and there are many more apart from Botox.

How Botulinum Toxin works?

Botox has its action on the muscle; it acts on SNAP 25 and doesn't let the muscle contract. It takes around 1.5-2 hours to bind to the nerve and it can take anywhere between 2 to 10 days to show full clinical effect. The effect of botox last from 3 months to 6 months varying individual to individual.

Some common Myths & Facts about Botulinum Toxin

Myth: It is new and not tested.

Fact: Botulinum toxin has a history of more than 20 years. And there are thousands of research papers available on the same.

Myth: It will give us botulism or else I will loose sensations and will have a plastic face.

Fact: None of it is correct. The lethal dose of botox is 2500 units to 3000 units which is 25 to 30 vials which is a lot of money to kill someone. A 100 unit vial of botulinum Toxin costs somewhere between Rs.18,000- Rs.20,000 INR

What do I have to know for getting good results from botulinum toxin?

For getting good results from botox injection one needs to understand the anatomy of the face or the areas where they want to inject botox.

How To Make Your Clinic A Patient Magnet?

How To Make Your Clinic A Patient Magnet?

Our clinics are supposed to be healing spaces rather than fear creating spaces. Patients are more scared to walk in a dental clinic than walk in a general hospital. Are we not ready yet to break that stereotype of being painful clinics??

Visit the DentistFriend.com dental portal to know dental courses all over India, to write blogs and participate in discussion forums

What’s in name??

Who does not like luxury? Add that extra x factor to your clinic. Have a nice innovative name. Now a days “spa” and “boutiques” are trending. Small addition may add that “chic” factor to your practice. Of course when you write spa…you need to do justice to the name. Have that kind of ambience that will confuse the patient if he in for a real spa!!

So what things incite fear in a patient and make our dental clinics an unpopular place to visit?

  1. 1. Pain, of course.

  2. 2. Hearing somebody cry or yell due to pain.

  3. 3. The typical hospital smell.

Painless dentistry- is not difficult to practice. Conscious sedation, oral sedation, magic wands for local anaesthesia….there are countless ways to do it!

Sound proofing??

Your dentistry may be painless for the mouth but you have to make it painless for the ear too! Some patients do complain of the hand piece noise that makes them irritable than any dental procedure. I have had to put on some earphones for few patients to calm them down and prevent them from hearing the hand piece noise. So what can we do??

  1. 1. Sound reduction hand pieces are available. They may be expensive but if you have clientele that demands such kind...why not??

  2. 2. Sound reduction headphones...they absolutely cut off any outside noise.

  3. 3. Scaring other patients?- You may use sound proof door and windows to prevent any chair side noise that may scare the patients in the waiting room

  4. 4. Increased insulation of walls and doors to absorb sound.

  5. 5. Planning a waiting area away from treatment room.

I smell fear!!!!!!

Leave alone patients, until I became a dentist, I would also relate going to a hospital with smell of spirit and I would feel nauseated. Most patients relate lot of situations with the smell. Many "people have a fight-or-flight reaction" to the sights, sounds, and smells of a dental office. This is the beginning of the fear development. To break that negative thought, air fresheners can be used. They not only create a pleasant environment but also reduce patient’s anxiety by calming the nerves. You can have orange, rose and lavender flavours which have shown to improve the patients mood and reduce anxiety levels (Trying to create a spa! Remember!)

What more??

Show an Waiting area-you can have some soothing photographs and music to relax the atmosphere rather than patient and procedure photographs.intra oral view of teeth to a normal regular non dental person. Most feel disgusted. I was once removed from my own family group for posting an intra oral picture of great work I did. But your patients don’t care about your good work in bold fully blown up pictures. So save them for consultation.

You can add in trendy magazines and board games for fun and this will be a distraction and help them pass time in the waiting. Fresh flowers or plants also help relax patients. Having plants or greenery around reduces pain perception. If you are an aquarium person, make sure your aquarium has clean water and no dead fish!

Temperature of the clinics- the patient should neither feel too cold nor too hot. The temperature should be well adjusted for them to feel at “home feeling”

Declutter your reception of unwanted stuff. There are so many things like pamphlets . These will help your area look neat and clean giving an impression of a well maintained hospital. There are many more simple ideas that can help you to make your clinic a patient magnet, for more of such innovative yet simple ideas stay tuned for part 2…………….Coming soon

Indemnity Insurance: Is Your Dental Practice Insured?

Indemnity Insurance: Is Your Dental Practice Insured?

In this blog post, I am going to discuss an essential topic that every doctor should be aware of. More often we read cases in the newspapers where a patient died or suffered negatively due to doctor’s negligence or patients or their relatives sued hospitals or doctors for their unprofessional conduct. These issues are very sensitive, due to some malpractices or negligence of doctors and also because some hospitals are trying to get more money by recommending unnecessary medical tests and treatments, people started looking at doctor’s community with distrust. It is found that there has been a rise in the patient’s awareness and the judiciary authorities are more consumer-friendly now and have been asking doctors or hospitals to compensate their mistakes.

According to a 2013 survey by Dr.Ashish Jha of Harvard School of Public Health, India recorded 5.2 million medico-legal cases in a year, for an incorrect prescription, wrong timing, and errors in surgery, incorrect drug, and dosage. Therefore, to protect doctors from the above financial risk, a special financial cover known as Professional Indemnity Insurance (PII) is introduced.

What is Professional Indemnity Insurance?

Doctors are the professionals who face the risk of financial liability while practicing their profession. The amount they can be sued can be very high. Therefore they need a special financial cover to protect themselves financially when they are sued for unprofessional conduct, negligence or mistakes during treatments.

In short, Professional Indemnity Insurance (PII) is an insurance policy specially meant for professionals like doctors, lawyers, Charted Accountants etc to cover liability falling on them as a result of errors, negligence, mistakes committed by them while rendering their professional service.

What does PII policy cover?

The policy covers all sums which the insured Doctor becomes legally liable to pay as damages to the third party in respect of any error or negligence committed while rendering professional service. Professional Indemnity Policy covers against following:

1. Professional Negligence

2. Unintentional breach of copyright or confidentiality

3. Loss of documents or data

4. Claim investigation costs

5. Offense & insult

6. Loss of goods or money

What PII does not cover?

1. Intentional damage

2. Claims and circumstances known before issuance of the policy

3. Act of Terrorism or war

4. Contractual liability

5. Insolvency of the insured

Which insurance companies provide PII for Doctors in India?

Insurance companies like United India Insurance, The New India Assurance Co. Pvt. Ltd., Reliance General Insurance, ICICI Lombard GIC Ltd, offer PII policy coverage for Doctors in India.

How to select the sum assured?

In professional Indemnity Policy, the sum insured is referred to as Limit of Indemnity. This limit is fixed per accident and per policy period which is called Any One Accident (AOA) limit and Any One Year (AOY) limit respectively.

The AOA limit, which is the maximum amount payable by the doctor for each accident, should be fixed depending upon the nature of treatment and the worst possible damage that could happen to the patient.

How to claim?

In case you feel that any event will likely to give rise to a liability claim, you should inform your insurance company immediately.

In case you receive any legal notice or summons, it should be sent to the insurance company. The company has the authority to arrange defense on your behalf.

The maximum amount payable will also include defense cost which will be included in AOA limit selected. The AOY limit will get reduced by the amount of claim paid for any one accident. Any number of such claims should not exceed the AOY limit.

How much does it cost?

Normally the premium for PII is 0.30% to 1 % of the amount insured. So if you wish to get a professional indemnity insurance for sum insured of 5 Lakh; then your premium may range from Rs.450 to 5000 The premiums may vary from company to company as well as depending on the doctor’s specialty, seniority, practice, equipment used etc. majorly. It will be a year policy, every year the doctor has to enter into a fresh agreement.

What if you don’t have a PII cover?

Indian patients treat doctors as next to God, but now the situation is changing. The fact is that Doctors are also humans who can commit mistakes during treatment. Hence it completely makes sense to have professional indemnity insurance to secure your dental/ medical practice.
